Cosmetic surgery is no longer a conversation happening primarily among women. More men are becoming comfortable exploring aesthetic treatments and procedures designed to help them look refreshed, confident, and more like themselves – and one procedure in particular continues to stand out: blepharoplasty, or eyelid surgery.
At Samra Plastic & Reconstructive Surgery, we have seen this shift firsthand. Over the past two years, our surgeons have noticed a growing number of male patients visiting our practices in Holmdel and Old Bridge, New Jersey, interested in improving the appearance of tired, heavy, or aging eyes.
And the trend goes far beyond our offices.
According to the American Society of Plastic Surgeons (ASPS), blepharoplasty has become one of the most popular cosmetic surgical procedures among men. In 2024 alone, more than 15,000 eyelid surgeries were performed on male patients, with men accounting for approximately 13% of all blepharoplasty procedures reported by ASPS. More recently, 2025 statistics showed eyelid surgery increasing by 19% overall, further demonstrating the growing interest in refreshing and rejuvenating the eye area.
So, why are more men choosing blepharoplasty?
The answer has a lot to do with changing attitudes toward male aesthetics, the desire for natural-looking results, and the fact that the eyes are often one of the first areas of the face to show signs of aging.
Blepharoplasty is a surgical procedure designed to address excess skin and/or fat around the upper or lower eyelids.
Depending on an individual’s anatomy and concerns, a patient may benefit from upper blepharoplasty, lower blepharoplasty, or a combination of both.
Upper blepharoplasty generally focuses on excess or drooping upper-eyelid skin that can make the eyes appear heavy, hooded, or tired. Lower blepharoplasty can address concerns such as prominent under-eye bags, excess skin, or fatty tissue beneath the eyes.
At Samra Plastic & Reconstructive Surgery, blepharoplasty is customized to the individual patient. Our surgeons evaluate each patient’s anatomy, goals, facial structure, and degree of aging before determining the most appropriate surgical approach.
For male patients in particular, that individualized approach can be especially important.
One of the most interesting changes we have seen in male cosmetic surgery is the way men describe what they want.
Very often, the goal isn’t, “I want to look 20 years younger.”
It’s:
That last point is particularly important.
Many male patients are looking for a subtle improvement rather than a dramatic transformation. They still want their face to look masculine. They still want to look like themselves. They simply want the person they see in the mirror to better reflect how energetic, healthy, or youthful they actually feel.
Blepharoplasty can be especially appealing because the goal isn’t to completely change the face. Instead, eyelid surgery can help address some of the specific signs of aging that make the face appear more tired or aged.
The skin surrounding the eyes is delicate, and changes in this area can become noticeable relatively early.
With age, genetics, sun exposure, and changes in the underlying tissues of the face, patients may begin to develop:
For some men, these changes are relatively mild. For others, genetics may cause noticeable upper-eyelid hooding or under-eye bags even at a younger age.
That means there isn’t necessarily a “right age” for male blepharoplasty.
The better question is whether the changes around your eyes bother you and whether surgery is an appropriate option for addressing them.
Male plastic surgery has become considerably more normalized.
Men are investing more in skincare. They’re getting neurotoxins. They’re exploring laser treatments, hair restoration, body contouring, gynecomastia surgery, facial procedures, and other aesthetic options that may have been less openly discussed a generation ago.
ASPS has noted an increase in men’s interest in aesthetic procedures and has specifically identified blepharoplasty as a leading facial cosmetic surgery for male patients. Experts have pointed to several possible reasons for the shift, including increased acceptance of cosmetic procedures, greater exposure to our own faces through video calls and social media, and more sophisticated surgical techniques tailored to male anatomy and aesthetic goals.
At Samra Plastic & Reconstructive Surgery, we have watched the same evolution take place locally.
Over the last two years especially, we’ve seen more men come into our Holmdel and Old Bridge offices simply wanting to have the conversation.
They aren’t necessarily looking for a dramatic makeover. Many are professionals, fathers, husbands, business owners, or retirees who feel great but notice that their eyes are making them look more fatigued or older than they feel.
And they want to know what their options are.
Male and female faces do not always age in exactly the same way, nor do patients necessarily have the same aesthetic goals.
With male blepharoplasty, maintaining a natural, masculine appearance is often a major priority.
Removing too much tissue or dramatically changing the shape of the upper eyelid may create a result that doesn’t match the patient’s facial features or goals. Instead, male eyelid surgery often requires a thoughtful, conservative approach that addresses the patient’s concerns while maintaining the characteristics that make his face look like his face.
This is why choosing an experienced, board-certified plastic surgeon matters.
A successful consultation isn’t simply about deciding whether someone “needs a blepharoplasty.” It is about understanding the patient’s anatomy, what is actually causing the concern, and what degree of correction will create a refreshed result without making the patient look overly operated on.
Not every male patient needs the same type of eyelid surgery.
An upper blepharoplasty may be appropriate for men bothered by excess skin or heaviness around the upper eyelids.
During upper-eyelid surgery, an incision is typically placed within the natural crease of the upper eyelid. Excess skin is removed, and depending on the patient’s anatomy, the surgeon may also address underlying muscle or fat.
Because of the natural location of the incision, the resulting scar is typically discreet as healing progresses.
Other patients aren’t particularly bothered by their upper eyelids at all.
Their primary complaint is the bags underneath their eyes.
Lower blepharoplasty can address excess skin and/or prominent fat beneath the eyes. Depending on the surgical technique and the patient’s needs, fat may be removed or repositioned, and incisions may be made externally or from inside the lower eyelid.
For men experiencing aging in both areas, an upper and lower blepharoplasty may be performed during the same surgery.
The goal is not to create a completely different eye shape. Instead, addressing both areas may create a more balanced and comprehensive rejuvenation of the eye area.
For many of our male patients, this is one of the biggest concerns.
And understandably so.
Most men who walk into a cosmetic surgery consultation aren’t hoping their coworkers immediately notice that they had surgery. What they usually want is for people to notice that they look good – without necessarily being able to figure out why.
That philosophy fits particularly well with blepharoplasty.
The goal should be a result that works with the rest of the face. Friends may think you look well-rested. Coworkers might notice that you look refreshed. Someone may ask whether you’ve lost weight, changed your routine, or recently taken a vacation.
But a well-planned blepharoplasty isn’t about giving you an entirely new face.
It’s about allowing your eyes to better reflect the way you actually feel.
Not every concern around the eyes requires surgery.
Nonsurgical treatments can be helpful for certain patients, particularly when signs of aging are relatively mild.
For example, neurotoxins such as Botox can soften certain dynamic wrinkles caused by muscle movement around the eyes. Skin resurfacing treatments, chemical peels, laser treatments, and skin-tightening procedures can also improve aspects of skin quality and mild laxity.
But there is an important distinction.
Nonsurgical treatments generally cannot remove significant excess eyelid skin or address prominent fat deposits in the same way surgery can.
Samra Plastic & Reconstructive Surgery notes that nonsurgical skin-tightening or resurfacing treatments may be appropriate for milder concerns, while surgery remains the most effective option for patients who need significant correction of excess eyelid skin or fat.
Sometimes the answer is nonsurgical treatment.
Sometimes it’s blepharoplasty.
And sometimes the best result comes from combining approaches.
That’s exactly why an in-person consultation is so valuable.
Recovery varies depending on whether a patient undergoes upper blepharoplasty, lower blepharoplasty, both procedures, or combines eyelid surgery with another facial procedure.
Swelling and bruising are expected during the early stages of healing. Many patients can begin returning to routine daily activities within approximately one to two weeks, although residual swelling and the final refinement of results can take longer.
Patients should remember that healing is a process. Following postoperative instructions, attending follow-up appointments, protecting the surgical area, and allowing the body adequate time to heal are all important parts of achieving a successful result.
Your surgeon will provide individualized recommendations based on your procedure and recovery.
Eyelid surgery may involve a relatively small area of the face, but even subtle changes around the eyes can have a major impact on your overall appearance.
Experience, judgment, and attention to detail matter.
At Samra Plastic & Reconstructive Surgery, our team includes five board-certified plastic surgeons: Said A. Samra, MD, FACS, FICS; Asaad H. Samra, MD, FACS; Munjal P. Patel, MD, FACS; Salem Samra, MD, FACS; and Fares Samra, MD, FACS. Our practice has provided plastic and reconstructive surgical care to patients throughout New Jersey for more than four decades.
Our surgeons perform a wide range of cosmetic and reconstructive procedures, including facial rejuvenation surgery, with treatment plans designed around each patient’s individual anatomy and goals.
And as interest in male cosmetic surgery continues to evolve, we’re seeing more men realize that there is nothing unusual about wanting to look as good as they feel.
In fact, Samra Plastic & Reconstructive Surgery’s own male before-and-after gallery includes multiple male blepharoplasty cases, reflecting the role eyelid surgery already plays within our male cosmetic surgery.
Perhaps the biggest reason male blepharoplasty continues to gain attention is that today’s male cosmetic patient isn’t necessarily looking for a dramatic transformation.
He wants to look refreshed.
He wants to look less tired.
He wants his appearance to match how he feels.
And he wants a result that still looks completely like him.
For the right patient, blepharoplasty can help accomplish exactly that.
If you’ve noticed heavy upper eyelids, persistent under-eye bags, loose eyelid skin, or a tired appearance that doesn’t reflect how you actually feel, a consultation can help determine whether upper blepharoplasty, lower blepharoplasty, a combination procedure, or a nonsurgical alternative may be right for you.
At Samra Plastic & Reconstructive Surgery, our board-certified plastic surgeons are proud to care for patients throughout Holmdel, Old Bridge, Monmouth County, Middlesex County, and communities across New Jersey.
As we’ve watched more male patients begin exploring eyelid surgery over the past two years, one thing has become increasingly clear: cosmetic surgery for men isn’t about looking like someone else.
It’s about feeling confident in the person looking back at you.
